CKD Workup

Standardized diagnostic approach for new-onset or progressive Chronic Kidney Disease.

1. Baseline Laboratory Evaluation
Essential tests for all patients with GFR < 60 or proteinuria

Chemistry & Hematology

  • BMP (Na, K, Cl, CO2, BUN, Cr, Glu, Ca)
  • Phosphorus, Magnesium, Albumin
  • CBC with differential
  • HbA1c (if diabetic or unknown)

Urine Studies

  • Urinalysis with Microscopy (sediment exam)
  • Urine Albumin/Creatinine Ratio (uACR)
  • Urine Protein/Creatinine Ratio (uPCR)
2. Serologic Workup (Glomerular Disease)
Indicated for hematuria, heavy proteinuria, or rapid decline

3. Renal Imaging
Gold Standard

Renal Ultrasound (Retroperitoneal)

Assess for kidney size (chronicity), cortical thickness, echogenicity, hydronephrosis (obstruction), and masses.

Note: Small echogenic kidneys (<9cm) suggest irreversible chronic disease. Large kidneys may suggest Diabetes, HIV, Amyloidosis, or PKD.
