Clinical Guidelines

Mild Hyponatremia (130-134 mEq/L):

  • โ€ข Usually asymptomatic
  • โ€ข Fluid restriction (500-800 mL/day)
  • โ€ข Identify and treat underlying cause

Moderate Hyponatremia (125-129 mEq/L):

  • โ€ข May have mild symptoms (nausea, headache)
  • โ€ข 0.9% or 3% saline depending on volume status
  • โ€ข Target correction: 4-6 mEq/L per 24h

Severe Hyponatremia (<120 mEq/L):

  • โ€ข Consider 3% hypertonic saline (513 mEq/L Na)
  • โ€ข Initial bolus: 100-150 mL over 10-20 minutes
  • โ€ข Can increase Na by 4-6 mEq/L acutely if symptomatic
  • โ€ข ICU monitoring mandatory
