Clinical Guidelines

Mild Hyponatremia (130-134 mEq/L):

• Usually asymptomatic
• Fluid restriction (500-800 mL/day)
• Identify and treat underlying cause

Moderate Hyponatremia (125-129 mEq/L):

• May have mild symptoms (nausea, headache)
• 0.9% or 3% saline depending on volume status
• Target correction: 4-6 mEq/L per 24h

Severe Hyponatremia (<120 mEq/L):

• Consider 3% hypertonic saline (513 mEq/L Na)
• Initial bolus: 100-150 mL over 10-20 minutes
• Can increase Na by 4-6 mEq/L acutely if symptomatic
• ICU monitoring mandatory
